The Anatomy of Subacromial Impingement
When the upper arm is abducted at 90 degrees to the torso (the "guillotine press" position), the greater tubercle of the humerus repeatedly compresses the supraspinatus tendon against the acromion process.
Tucking the elbows slightly to a 45 to 60-degree angle relative to the ribcage opens the subacromial space, eliminating bone-on-tendon friction while aligning the chest fibers along their natural line of pull.
